RESUMO

The Covid-19 pandemic has introduced the world to a new chapter in the last three to four years. The focus of this review is on a significant but often overlooked group and topic that has received limited research attention. Recent studies show that the psychological impact of the COVID-19 pandemic is still intense, even around three years later. This article will summarize and discuss the results of 52 studies on anxiety, depression, post-traumatic stress symptoms (PTSS), fear of loneliness, suicidal ideation, and resilience in the elderly population before and after the SARS-CoV-2 pandemic. In addition to detrimental effects, attention is also given to the improved coping abilities and lack of significant psychological distress among the geriatric population when compared to younger age groups. This review will encompass research conducted on both the population of Bangladesh and the global population as a whole.

A 42 day factorial trial (3x2) was designed to evaluate the effect of short-term starvation with different feeding frequencies on performance, feed utilization, physiological status and appetite responses of red hybrid tilapia fingerlings. Eighteen plastic tanks with a capacity of (55 L) were used to accomplish this work. Fingerlings with an average initial weight of 23 g ± 0.2 (SE) were randomly stocked at a rate of 8 fingerlings/aquarium. Six groups were designated as the following: II/ED: fish was fed twice every day; IV/ED: fish fed four times every day; II/EOD: fish fed twice every other day (alternate-day feeding or one day of feeding followed by another of fasting); IV/EOD: was fed four times every other day; II/EO3D: fish fed twice every other three days (three day of feeding followed by another of fasting) and IV/EO3D: fish fed four times every other three days. Fish were fed on commercial diets 30 % protein (4 % of biomass). Results showed insignificant differences between fish fed every other day and those fed every day in some growth indicators. In the same trend, the interaction between feed deprivation and feeding frequency cleared that fingerlings of IV/EOD did not significantly differ with those fed every day in growth indices. Moreover this treatment was the best in feed conversion efficiency and several physiological indicators.

Since the declaration of the coronavirus disease 2019 pandemic, all efforts were directed towards limiting the transfer of the disease and preventing severe disease forms from occurring. In this regard, numerous vaccines were quickly developed to limit the associated morbidity and mortality of the disease and to reduce the burden on healthcare systems worldwide. However, to date, vaccine hesitancy remains a major limitation to vaccine distribution, with varying degrees in different countries. Therefore, the authors conducted this literature review to highlight the magnitude of this issue throughout the globe and summarize some of its major causes (i.e. governmental, healthcare system-related, population-related, and vaccine-related) and contributing factors (i.e. knowledge/awareness, social media, etc.). In addition, the authors highlighted some of the main motivating factors that can minimize the burden of vaccine hesitancy at the population, governmental, and worldwide levels. These include structural (i.e. government, country), extrinsic (i.e. family, friends), intrinsic (i.e. self-perception), and other factors (financial and nonfinancial). Finally, the authors proposed some implications for future research to ease the vaccination process and hopefully, put an end to this problem.

This article seeks to decentre the Global North knowledge production about 'work-life balance' (WLB) in academia by applying a temporal gaze to illuminate WLB possibilities in Bangladeshi academia where institutional WLB policies are absent. Drawing on Adam's (2008) timescapes and Flaherty's (2002) time work concepts, we focus on Bangladeshi women faculty's experiences as an example of how a temporal gaze can help illuminate the interrelationships between time, gender, and life transitions underlying women faculty accounts of WLB in a Global South context. Drawing on the narratives of three Bangladeshi women faculty in different career stages and family statuses, we probe how women faculty manipulate, control, or customize their temporal experience (i.e. temporal agency) in response to local gendered norms and life transitional episodes (e.g. separation, academic mobility, illness, and/or retirement). We demonstrate how WLB is not a static outcome, but a work-in-progress, and that a temporal lens helps illuminate multiple time work strategies that emerge during life transitional episodes. We argue that a temporal lens troubles the outcome (quantitative, clock-oriented) and spatial orientation of WLB practices, as our participants constantly blurred work/home boundaries refracted across social positionality, gendered norms, and relationships. By examining the temporal dimensions underlying WLB, we contribute a comprehensive understanding of the interrelationships between academic/personal life, various roles, and temporality in a South Asian context.

Burn injury is still a life-threatening event, associated with high mortality and morbidity inspite of recent advances and infection control practices. This cross sectional study was conducted at Microbiology department of Rajshahi Medical College (RMC), Bangladesh with the objective to provide an insight into the current trend of aerobic bacteria with their antibiogram in burn wound infected patients admitted in Burn and Plastic Surgery Unit of RMCH from 1st January 2016 to 31st December 2016. A total of 212 wound swabs were collected and processed as per standard protocol. The isolates were identified by standard microbiological techniques and antimicrobial susceptibility was performed by Modified Kirby-Bauer disk diffusion method. Burn injurywas highest (44.81%) in age group 21-30 years in fire related burn patients (41.98%) and female were predominant (59.91%). Among 212 samples 89.62% yielded growth and 196 bacterial species were isolated in which P. aeruginosa were the predominant organism (34.18%) followed by Proteus spp. (23.47%), Klebsiella spp. (14.80%), E. coli (11.73%), S. aureus (10.71%), Acinetobacter spp. & CoNS (2.04%) and Enterobacter spp. (1.03%). In this study meropenem, amikacin and vancomycin were highly sensitive drugs but 3rd generation cephalosporin and ciprofloxacin showed resistance. Therefore continuous microbiological surveillance and careful in vitro testing prior to antibiotic treatment is required to reduce the emergence of multidrug resistant pathogens.

Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a rare disease with an unknown etiology which most commonly results in subacute diplopia and ataxia. Diagnosis is achieved through a triad of the following findings: lymphocytic pleocytosis with increased CD4+ T cells on cerebrospinal fluid (CSF) analysis; perivascular punctate and curvilinear hemorrhages in the pons, medulla, or cerebellum on magnetic resonance imaging (MRI) with contrast; and the cessation of symptoms after the initiation of corticosteroids. Here, we report the case of a 23-year-old male who presented with non-specific signs and symptoms, including diffuse weakness in all limbs, ataxia, and slurred speech. The diagnosis was achieved through a contrast MRI of the brain, suggestive of brainstem encephalitis, and a CSF analysis, which revealed elevated glucose and protein levels. Intravenous methylprednisolone was administered for five days and resulted in acute improvement of the patient's clinical status. Repeat CSF analysis and MRI of the brain with contrast two weeks later showed resolution of previous findings. CLIPPERS syndrome is a newly identified disease thought to cause a predominantly inflammatory reaction in the pons, medulla, cerebellum, and supratentorial region. MRI with contrast tends to reveal a "salt and pepper appearance" in a punctate and curvilinear fashion. The hallmark of treatment is corticosteroid therapy, and discontinuation of therapy should be done with caution as relapse of the syndrome with corticosteroid withdrawal has been documented.

OBJECTIVES: Perception of spectrally degraded speech is particularly difficult when the signal is also distorted along the frequency axis. This might be particularly important for post-lingually deafened recipients of cochlear implants (CIs), who must adapt to a signal where there may be a mismatch between the frequencies of an input signal and the characteristic frequencies of the neurons stimulated by the CI. However, there is a lack of tools that can be used to identify whether an individual has adapted fully to a mismatch in the frequency-to-place relationship and if so, to find a frequency table that ameliorates any negative effects of an unadapted mismatch. The goal of the proposed investigation is to test the feasibility of whether real-time selection of frequency tables can be used to identify cases in which listeners have not fully adapted to a frequency mismatch. The assumption underlying this approach is that listeners who have not adapted to a frequency mismatch will select a frequency table that minimizes any such mismatches, even at the expense of reducing the information provided by this frequency table. DESIGN: Thirty-four normal-hearing adults listened to a noise-vocoded acoustic simulation of a CI and adjusted the frequency table in real time until they obtained a frequency table that sounded "most intelligible" to them. The use of an acoustic simulation was essential to this study because it allowed the authors to explicitly control the degree of frequency mismatch present in the simulation. None of the listeners had any previous experience with vocoded speech, in order to test the hypothesis that the real-time selection procedure could be used to identify cases in which a listener has not adapted to a frequency mismatch. After obtaining a self-selected table, the authors measured consonant nucleus consonant word-recognition scores with that self-selected table and two other frequency tables: a "frequency-matched" table that matched the analysis filters with the noisebands of the noise-vocoder simulation, and a "right information" table that is similar to that used in most CI speech processors, but in this simulation results in a frequency shift equivalent to 6.5 mm of cochlear space. RESULTS: Listeners tended to select a table that was very close to, but shifted slightly lower in frequency from the frequency-matched table. The real-time selection process took on average 2 to 3 min for each trial, and the between-trial variability was comparable with that previously observed with closely related procedures. The word-recognition scores with the self-selected table were clearly higher than with the right-information table and slightly higher than with the frequency-matched table. CONCLUSIONS: Real-time self-selection of frequency tables may be a viable tool for identifying listeners who have not adapted to a mismatch in the frequency-to-place relationship, and to find a frequency table that is more appropriate for them. Moreover, the small but significant improvements in word-recognition ability observed with the self-selected table suggest that these listeners based their selections on intelligibility rather than some other factor. The within-subject variability in the real-time selection procedure was comparable with that of a genetic algorithm, and the speed of the real-time procedure appeared to be faster than either a genetic algorithm or a simplex procedure.

Mutations in pancreatic duodenal homeobox 1 (PDX-1) can cause a monogenic form of diabetes (maturity onset diabetes of the young 4) in humans, and silencing Pdx-1 in pancreatic ß-cells of mice causes diabetes. However, it is not established whether epigenetic alterations of PDX-1 influence type 2 diabetes (T2D) in humans. Here we analyzed mRNA expression and DNA methylation of PDX-1 in human pancreatic islets from 55 nondiabetic donors and nine patients with T2D. We further studied epigenetic regulation of PDX-1 in clonal ß-cells. PDX-1 expression was decreased in pancreatic islets from patients with T2D compared with nondiabetic donors (P = 0.0002) and correlated positively with insulin expression (rho = 0.59, P = 0.000001) and glucose-stimulated insulin secretion (rho = 0.41, P = 0.005) in the human islets. Ten CpG sites in the distal PDX-1 promoter and enhancer regions exhibited significantly increased DNA methylation in islets from patients with T2D compared with nondiabetic donors. DNA methylation of PDX-1 correlated negatively with its gene expression in the human islets (rho = -0.64, P = 0.0000029). Moreover, methylation of the human PDX-1 promoter and enhancer regions suppressed reporter gene expression in clonal ß-cells (P = 0.04). Our data further indicate that hyperglycemia decreases gene expression and increases DNA methylation of PDX-1 because glycosylated hemoglobin (HbA1c) correlates negatively with mRNA expression (rho = -0.50, P = 0.0004) and positively with DNA methylation (rho = 0.54, P = 0.00024) of PDX-1 in the human islets. Furthermore, while Pdx-1 expression decreased, Pdx-1 methylation and Dnmt1 expression increased in clonal ß-cells exposed to high glucose. Overall, epigenetic modifications of PDX-1 may play a role in the development of T2D, given that pancreatic islets from patients with T2D and ß-cells exposed to hyperglycemia exhibited increased DNA methylation and decreased expression of PDX-1. The expression levels of PDX-1 were further associated with insulin secretion in the human islets.

OBJECTIVE: To define the interval between clinical presentation of necrotizing enterocolitis (NEC) and bowel perforation in neonates. METHODS: Charts of neonates with discharge diagnosis of NEC (n = 124) from our NICU during 2004-2008 were retrospectively reviewed. Demographic data were collected. Acute episode of NEC was defined as the interval between clinical presentations to resumption of enteral feeds. Neonates are followed, as a standard of care, clinically and radiologically until resumption of enteral feeds at the discretion of the attending clinician. Abdominal radiograph results were reviewed serially to determine the interval between clinical presentation and bowel perforation using pneumoperitoneum as the surrogate radiological marker. Histological report of resected bowel specimens was reviewed for coagulative necrosis as evidence of NEC and to exclude spontaneous intestinal perforation (SIP). Neonates with stage 1 NEC and SIP were excluded from the results. RESULTS: 105 neonates with stage 2 NEC were included in the study. Forty-six needed surgical treatment (group 2) and 59 did not need surgery (group 1). Twenty-six (26/46, 56%) group 2 neonates had bowel perforation and hence required surgery. Pneumoperitoneum was noted at a median interval of 1 day after presentation of symptoms. Twenty neonates in group 2 needed surgery for clinical indications including worsening clinical examination, thrombocytopenia or persistent metabolic acidosis. Fifty-nine neonates (group 1) were treated with bowel rest, antibiotics and parenteral nutrition. Group 2 neonates were significantly more premature, weighed less and had less radiographs than group 1 neonates. Mortality was significantly higher in group 2 compared to group 1. CONCLUSION: Bowel perforation occurs at a median interval of 1 day after clinical presentation of NEC. Neonates not needing surgery for their disease are exposed to significantly more radiographs than those needing surgery. Radiological evaluation can be safely minimized or eliminated after 2 days of presentation.

RESUMO

Cochlear implants can restore hearing to deaf individuals by electrically stimulating the auditory nerve. They do so by assigning different frequencies to different stimulating electrodes via a frequency map. We have developed a device that enables us to change the frequency map in real time. Here, in normal-hearing adults listening to an acoustic simulation of a cochlear implant, we investigate what frequency maps are initially preferred, and how the ability to understand speech with that preferred map compares with two other maps. We show that naive listeners prefer a map that balances the need for low-frequency information with the desire for a naturally-sounding stimulus, and that initial performance with this listener-selected map is better than that with a map that distorts the signal to provide low-frequency information.
